This can be separate from your mailing address and will also be printed in the directory along with your mailing and permanent addresses.

Please and send back the form along with 2 passport size colour photos which will also be published in the directory..

Write your name and membership number at the back of ONE of the photos.

In case you have forgotten your membership number leave that column blank.
